At the start of each cadence, check whether last cycle's committed actions happened. Name it plainly when they did not. Keep the tone constructive: the purpose is to surface and clear stuck work, not to assign blame. A slipped commitment is a signal to unblock, re-own, or re-scope, not to punish.
