Countermeasure and Problem-Solving Protocol.

Design and implement a fix that removes a root cause so the problem cannot return, and run root-cause problem-solving as a standing discipline rather than a one-time effort.

Section A: Design the countermeasure

For the root cause identified, design the fix that removes it.

If it changes the process structurally, route the new way into standardization so the better way becomes the documented standard.

Section B: Run it as a standing discipline

Make root-cause problem-solving an ongoing capability, not a one-time cleanup. Keep the problem log live so new recurring problems get caught.
