Requesting feedback and then doing nothing teaches people that candor is pointless and deepens disengagement. The "how we told people" column is what makes the whole system credible: visible action closes the loop.

Reading the result: if only reassurance surfaces, the channels are not yet safe, revisit what makes honesty safe. Visible action on what you hear is what makes people keep telling you the truth.
