The recurring shortlist. A handful of problems usually account for most of the firefighting. Name them, because those, not the novel ones, are where root-cause work pays off:

Section B: Confirm the fix held

After a countermeasure is in place, track the problem over time to confirm it genuinely stopped, rather than went briefly quiet. If it returns, the root cause was not the real one, or the fix did not remove it. Cycle back to root-cause analysis and dig deeper.
