One row per defect or rework event. The most diagnostic field is Stage caught: a defect caught by the client is the expensive kind, and the kind your gates should be pulling upstream.

On a regular cadence, scan for the same defect type recurring. A defect that appears more than once is a systemic gap, not bad luck.
