Get an honest read of how much trust and psychological safety exist, so collaboration work builds on a real diagnosis, and re-read it to sustain trust over time.
Assess each dimension from behavior, not just from what people say to your face. Rate each Strong, Mixed, or Weak, and note the evidence you actually observe. Silence is the dangerous reading: a team that never disagrees, never admits mistakes, and never asks for help is not harmonious, it is unsafe.

If no one ever pushes back or admits a mistake, do not read that as a healthy, agreeable team. Read it as low safety: the lack of safety is exactly what keeps people from telling you it is missing.
Reading the result: weak dimensions are where the collaboration work concentrates. At Step 5 of the guide, re-read this periodically, trust is lost quickly and a single unaddressed breach can undo months of work.
